We may also receive information from My Health Record, hospitals, specialists, pathology providers, diagnostic imaging providers and other healthcare professionals involved in your care.

We collect your information to provide safe and effective healthcare, maintain accurate medical records, arrange referrals and investigations, communicate with you about your healthcare, process billing and meet legal and regulatory requirements.

Your information may be shared with healthcare providers involved in your care, when preparing referrals on your behalf, or where required or authorised by law.

We may also disclose information when necessary to prevent a serious threat to life, health or safety, or for mandatory public health reporting.

We will not disclose your information to third parties for marketing purposes or overseas without your consent unless permitted by law.

You may request access to your medical records at any time. Requests must be made in writing and will be assessed in accordance with applicable privacy legislation.

Administrative fees may apply for processing and providing copies of records.
